Diastasis Recti Isn’t Just a Women’s Issue: What Men Need to Know

For most people, the term "diastasis recti" evokes a very specific image: a postpartum woman rebuilding her core after childbirth. It is a legitimate association, but an incomplete one.
In clinics like ours, we are seeing more and more men present with the same condition. Active adults. Weightlifters. Men in their 40s and 50s who have noticed a change in their midsection they cannot explain, or who are experiencing core weakness, back pain, or a subtle bulging along the front of their abdomen that does not respond to the usual answers.
If any of that sounds familiar, this piece is for you. Not to alarm you. Just to explain what may be happening, what the research says, and what the path forward looks like when the condition is understood clearly.
What Is Diastasis Recti?
Diastasis recti is a separation of the two halves of the rectus abdominis, the paired muscle running vertically down the front of the abdomen that creates the visible "six-pack" appearance. Between those two halves runs a band of connective tissue called the linea alba, which normally holds them close together.
When intra-abdominal pressure exceeds what the linea alba can manage over time, that connective tissue stretches. The two sides of the rectus abdominis drift apart. What was a tight, coordinated system becomes a wider, less structurally sound one.
To be clear about what this is and is not: diastasis recti is not a muscle tear. The rectus abdominis itself remains intact. It is a change in the connective tissue between the two halves. It is also not automatically a hernia, though the two conditions can occur together. The distinction matters for how the condition is understood and treated.
Can Men Really Get Diastasis Recti?
Yes. Men can absolutely develop diastasis recti, and the condition is more common than most people realize. It is simply less recognized because the medical conversation has centered so heavily on pregnancy and postpartum recovery.
The mechanism, however, is not specific to pregnancy. Anything that produces sustained or repetitive intra-abdominal pressure beyond what the connective tissue can manage can create separation over time. In men, the most common contributors include:
Chronic elevated intra-abdominal pressure. Carrying excess weight around the midsection places continuous outward pressure on the abdominal wall. Over years, that pressure gradually stretches the linea alba.
Improper lifting mechanics. Repetitive heavy lifting done with poor bracing, particularly with breath-holding that maximizes downward pressure without balancing it across the whole abdominal cylinder, is a common contributor. This is why we see diastasis in serious lifters as well as sedentary men.
Chronic coughing. Long-term respiratory conditions that produce sustained coughing generate repeated pressure spikes through the abdominal wall.
Post-surgical changes. Abdominal surgeries can affect the integrity of the linea alba and surrounding fascia, sometimes creating conditions for separation to develop.
Aging and connective tissue changes. Connective tissue integrity naturally shifts with age, particularly when combined with any of the above factors.
Most men with diastasis have some combination of these contributors layered over time. The condition rarely has a single cause. It is a story of accumulated pressure that eventually exceeded what the tissue could hold.
Signs and Symptoms of Diastasis Recti in Men
The presentation varies significantly, which is one of the reasons this condition goes underdiagnosed. Some men have visible signs and no functional complaints.

Others have significant functional issues with only subtle visible changes. The symptoms most worth knowing about include:
Abdominal doming or coning. A visible ridge or dome down the center of the abdomen when sitting up from a lying position, doing a crunch, or straining. Often the most recognizable sign.
A midline bulge that appears with pressure. Something that pushes forward when you cough, lift, or bear down, and settles when you relax.
Core weakness that seems out of proportion. A sense that your midsection is not "connecting" the way it used to. Difficulty engaging your core during lifts. A lack of the trunk stability you rely on for athletic movement.
Lower back pain that keeps returning. When the anterior core is not managing pressure and load well, the lumbar spine often absorbs the difference. Chronic low back pain unresponsive to typical treatment is one of the most common ways diastasis surfaces in men.
Reduced performance in lifting or sport. Numbers that used to feel manageable now feel harder. The bracing strategy that used to work no longer produces the same stability.
Postural changes. A gradual forward tilt of the pelvis, an increase in lumbar curvature, or a change in how your midsection sits at rest.
These symptoms do not appear together in a neat pattern. One man may notice only doming during workouts. Another may have significant back pain and no visible signs. This variability is exactly why a movement assessment is often the most reliable way to identify what is going on.
Why It Often Goes Undiagnosed
Most men with diastasis do not know they have it. They have explanations that seem reasonable and never think to look further. The midsection change is just aging. The "dad bod" is a natural evolution. The weak core is a fitness problem to be solved with more crunches. The recurring back pain is a spine issue, unrelated to anything happening in front.
None of these interpretations are unreasonable. They are just often incomplete. The anterior abdominal wall plays a central role in how the whole trunk manages pressure and produces stability. When it is compromised, the effects show up in ways that are easy to explain with something else, especially when the possibility of diastasis has never been raised in the conversation.
There is no shame in having missed the signs, and no fault in the visits where it was not identified. This condition is simply not part of the standard health conversation for men. Once you know what to look for, you can start asking the right questions.
Diastasis Recti vs. Hernia: Understanding the Difference
Because these two conditions are frequently confused, and sometimes coexist, it is worth understanding how they differ.

The two conditions can occur together, which is one reason a comprehensive evaluation matters. If you have any suspicion of a hernia, particularly one that is tender, growing, or non-reducible, that warrants medical evaluation independently.
Can Exercise Actually Make Diastasis Recti Worse?
Yes. Certain exercises done with poor form or inappropriate loading can worsen the condition, and this is important for active men to understand because the instinct with core weakness is usually to do more traditional core work.
The exercises most commonly implicated include high-repetition crunches and sit-ups with poor bracing, aggressive rotational movements loaded beyond what the core can manage, heavy lifting done with breath-holding strategies that concentrate pressure downward, and any exercise producing visible doming or coning. That doming is a real-time signal that the anterior core is failing to manage the pressure being generated.
The reason is straightforward. The linea alba is already stretched. Repeatedly generating pressure that pushes outward on already-compromised tissue reinforces the pattern rather than reversing it.
This does not mean avoiding exercise. It means rebuilding the pressure management and deep core coordination first, then progressively returning to loading in ways that support the abdominal wall rather than continue stressing it.
How Physical Therapy Actually Helps
Effective rehabilitation is not a matter of prescribing a generic list of exercises. It is a systematic process of restoring the coordinated function of the entire pressure management system, of which the rectus abdominis is only one component.
The process begins with a thorough assessment. How does the abdominal wall respond to different demands? What is the breathing pattern? Where is pressure being generated and where is it being managed poorly? What compensations have developed in the surrounding movement system? How does the diastasis behave under load and during the activities that matter to the individual?
Treatment then focuses on several interconnected elements. Breathing mechanics are foundational. The diaphragm, when working properly, contributes significantly to pressure regulation and creates the neuromuscular foundation for deeper core activation. Retraining breathing is often the first step and one of the most impactful.
Pressure management strategies come next. This includes learning to brace in ways that distribute pressure evenly around the trunk rather than concentrating it against the linea alba, and knowing when and how to modulate intra-abdominal pressure during different activities.
Deep core activation focuses on the transverse abdominis and its coordination with the pelvic floor, the diaphragm, and the multifidus muscles of the spine. These systems function together, and rebuilding their coordination is central to lasting improvement.
Progressive loading gradually reintroduces functional demands, from bodyweight movements to loaded carries to sport-specific tasks. For active men who want to return to lifting or sport, this progression is where rehabilitation stops feeling like a limitation and starts feeling like a rebuild.
Exercises That May Help (And Why the Right Progression Matters)
Rather than prescribe a home program, which would be inappropriate without individual assessment, it is more useful to understand the categories of movement that typically feature in diastasis rehabilitation.
Diaphragmatic breathing is nearly always the starting point. Slow, deep breaths that expand the ribcage laterally and posteriorly rather than pushing the belly outward reestablish the foundational relationship between breath and core function.
Transverse abdominis activation reintroduces the deep stabilizing layer that is often disengaged in men with diastasis. Learning to feel and consciously activate the TVA is a skill that takes practice to translate into automatic function.
Dead bug progressions build the coordination between limb movement and trunk stability without loading the anterior core in ways that produce doming.
Bird dogs and quadruped variations develop rotational stability, cross-body coordination, and the trunk control needed for functional movement.
Loaded carries retrain the core to manage load in upright positions that mirror real-life demand.
Functional strengthening progressively integrates trunk stability with the movements the individual needs to perform, whether that is lifting, running, or specific sport demands.
Every one of these categories can be done well or poorly. The right progression, loading, and cueing depend entirely on the individual, which is why targeted rehabilitation produces meaningfully better outcomes than generic programs.
Can Diastasis Recti Heal Without Surgery?
For most men, yes. Significant improvement is possible with conservative rehabilitation, particularly when the underlying contributors are addressed alongside the diastasis itself.
Surgical repair is generally reserved for cases involving severe functional limitation, significant associated hernia, cosmetic concerns important to the individual, or situations where conservative care has been thoroughly attempted without adequate improvement.
The realistic expectation is that most men can meaningfully improve symptoms, restore functional capacity, and return to the activities they care about through movement-based rehabilitation. Whether the visible separation fully closes varies by individual, and function often improves substantially even when some anatomical separation remains.
The Questions Men Ask Most
A few questions come up often enough that they are worth addressing directly.
Can weightlifting cause diastasis recti? It can contribute, particularly when heavy lifting is combined with poor bracing mechanics and aggressive breath-holding. Lifting itself is not the enemy. The way pressure is managed during lifting is what matters.
Can you still lift weights with diastasis recti? In most cases, yes, though the approach and progression need to be adjusted. Many men return to serious lifting after rebuilding their pressure management and core coordination. The path back is individualized.
How long does recovery take? Meaningful functional improvement often appears within weeks of committed, targeted rehabilitation. Structural changes and full return to loading typically take months. This is not a quick fix. It is a rebuild, and the timeline reflects that.
Can you have visible abs with diastasis recti? Yes. Muscle definition is possible even when some separation remains. Lean body composition and diastasis are not mutually exclusive.
Will it come back if I stop rehabilitating? The underlying contributors, particularly bracing patterns and pressure management strategies, need to become integrated into how you move long-term. Returning to the habits that contributed to the condition can reintroduce the same stresses. This is why rehabilitation ultimately becomes about movement education, not just exercise prescription.
When Should You See a Physical Therapist?
An evaluation is worth considering if you notice visible doming or a midline ridge when engaging your core, persistent core weakness that has not responded to standard training, chronic lower back pain that seems disconnected from any specific event, difficulty returning to lifting or sport at the level you used to, or a midsection change that has developed over time and does not fit the usual explanations.
A movement assessment with a physical therapist trained in this area will identify whether diastasis is present, evaluate the surrounding movement patterns that may be contributing, and outline a path forward that fits your specific situation and goals.
